The Rounding section of Charge Capture is arguably the most useful and most powerful section of the application. It utilizes the Encounters section of the application in an orderly, date/facility oriented way to mimic the act of rounding. In a quick glance, the section shows you who you need to round on and whether you have entered charge data on that person.
Selecting the '+' button at the top-left of the application allows a new Rounding item to be entered. The date is automatically set to today, but may be changed to enter a previous date for an encounter. Charge (cpt) and diagnosis (icd-9) data are entered as in the Encounters section.
Entries within the Rounding list are color coded to indicate the completion status:
• A white colored list item has no data entry at all. The Encounters section will not have an entry for the person/facility shown within that item. As soon as the user taps on a white entry, the specific data can be entered and an encounter will be created.
• A red colored list item indicates that incomplete data has been entered. Either charge (cpt) or diagnosis (icd-9) data is missing from the encounter.
• A green colored list item indicates that data for charge and diagnosis items have been entered.
Patient's are discharged from the Rounding list in several possible ways. The first is automatic in that selecting a CPT code marked as a "discharge code" will discharge the patient from the list. The 'dc' icon will appear within that list item and new Rounding items for following days will no longer be created. If the "delete post discharge" setting (see Settings) is marked as 'ON', any Rounding item already on the list after the day of discharge will be automatically deleted.
Example: A patient was seen on a Monday and a Rounding item was entered for that day. The patient was discharged from the hospital the next day, but for unnamed reasons, no entries were made in the Rounding section of the program at that time. You do enter the section on Thursday and Rounding items are automatically entered for Tuesday through Thursday. If you enter the encounter data for Tuesday and select a CPT code with a "discharge identifier", the Rounding items for Wednesday and Thursday will be automatically deleted.
The second method for discharging a patient can be found within the Rounding Encounter detail view (see screenshot #2). Slide the 'Discharged' slider at the bottom of the view to "YES" to accomplish this. Note that the patient is immediately marked as discharged and any entries in the rounding list beyond the date in question will be automatically deleted as discussed above. This cannot be undone once it occurs. The slider can be changed to "NO" to un-mark the patient as discharged. Entries after this date will then be auto entered again as marked in the Settings section.
Example: As in the example above, a patient has been in the hospital and previous charges have been entered. The last time the patient was seen was a Friday. It is now Tuesday and the patient was discharged by one of your call partners over the weekend. Thus, unlike the example above, you cannot personally enter a discharge CPT code in order to remove the patient from the rounding list. In this case, you may go into the last appropriate charge (Friday in this case) and mark the 'Discharged?' item in the Rounding Encounter detail view (see screenshot). All entries after that date will be removed from the Rounding list.
A rounding item can be removed from the rounding list as any other list item. The "swipe" gesture can be used on a rounding item to access the buttons for "Discharge" and "Not Seen". Selecting discharge will accomplish the same as marking the patient as discharged within the encounter detail section as described above. Tapping the 'Not Seen' button will remove that rounding item from the list. It is important to note that this does not "discharge" the patient from within the Rounding section. Charge Capture will continue to add new rounding items on subsequent days until the patient in question is discharged by one of the means discussed above.
Example: A patient is rounded on during a Friday session and charges entered for that day. A call partner rounds on that patient on Saturday and Sunday. When the rounding section is opened over the weekend, new items are entered for Saturday and Sunday. If desired, iPhone "swipe gestures" through the unwanted rounding items and selection of the "Not Seen" button can be made so that they are removed from the rounding list (see screenshot #3). Charge Capture will still add subsequent rounding items (e.g., on Monday) so that charges for this patient can be entered.
Finally, mass markings of patients can be done in Edit mode of the Rounding tab. While in edit mode, select the items to be marked. Tap the "Mark" button in the toolbar at the bottom of the screen, the select either "Discharge" or "Mark as Unseen" from the resulting dialog. This will accomplish the selection across all selected rounding items and cannot be undone, thus this process is to be used carefully.
